Multi-modal Characterisation of Gastroenteropancreatic Neuroendocrine Tumours (GEP-NETs) Treated With Targeted Radionuclide Therapy (TRT): Prospective Interventional Multicentre National Cohort
Operandi project aims to address unmet clinical needs in the current management of GEP-NETs treated with PRRT by exploring new opportunities provided by imaging-based (AI algorithms) and data augmentation, simultaneous 68Ga-DOTATOC PET-MRI imaging, and novel approaches to increase patient selection and PRRT efficacy (genomic profiling, radiopotentiators, and new radionuclides). The study aim to identify predictive and early markers indicative of PRRT effectiveness based on a large prospective cohort of GEP-NET patients. This cohort will be used to uncover relevant predictive signatures within the morphological, functional, and molecular imaging data using novel imaging-based AI approaches with a new patient imaging pathway including simultaneous 68Ga-DOTATOC PET-MRI. Considering this global objective, the objective of this clinical research protocol is to provide clinical, molecular and imaging data in a prospective standardized study, notably by performing systematic PET-MRI at baseline, at mid course of PRRT and 1 year after PRRT initiation, in patients with advanced GEP-NETs treated with PRRT.
Le projet Operandi vise à répondre aux besoins cliniques non couverts dans la prise en charge actuelle des GEP-NET traitées par radiothérapie interne vectorisée par récepteurs peptidiques (PRRT), en explorant de nouvelles perspectives offertes par l'imagerie assistée par algorithmes d'intelligence artificielle (IA) et l'augmentation de données, l'imagerie simultanée TEP-IRM au 68Ga-DOTATOC, ainsi que de nouvelles approches visant à améliorer la sélection des patients et l'efficacité de la PRRT (profilage génomique, radiopotentialisateurs et nouveaux radionucléides). L'étude vise à identifier des marqueurs précoces et prédictifs de l'efficacité de la PRRT, à partir d'une large cohorte prospective de patients atteints de GEP-NET. Cette cohorte permettra de mettre en évidence des signatures prédictives pertinentes au sein des données d'imagerie morphologique, fonctionnelle et moléculaire, grâce à de nouvelles approches d'IA appliquées à l'imagerie, avec un nouveau parcours d'imagerie des patients incluant une TEP-IRM simultanée au 68Ga-DOTATOC. Au regard de cet objectif global, l'objectif de ce protocole de recherche clinique est de fournir des données cliniques, moléculaires et d'imagerie dans le cadre d'une étude prospective standardisée, notamment en réalisant une TEP-IRM systématique à l'inclusion, à mi-parcours de la PRRT et 1 an après l'initiation de la PRRT, chez des patients atteints de GEP-NET avancées traitées par PRRT.
