Study of the Serotype and Genotype of BK Virus in Kidney Transplant Recipients and Their Donors to Identify Individuals at Risk of Nephropathy

The aim of this observational study is to characterize the urinary replication of BK polyomavirus (BKV) in kidney transplant recipients. Although BKV reactivation after transplantation is well established, the origin of the replicating virus remains uncertain. Current evidence suggests that BKV detected in recipients may originate either from the transplanted kidney (donor-derived) or from viral reactivation in the recipient. The evaluation of new biomarkers to predict BKV replication are needed. This study seeks to address the following key questions: * Origin of the replicating virus: Is the BKV detected in the recipient identical to the virus originating from the donor kidney? * Host immune response and viral genotype: Is there an association between the recipient's immune response and the genotype of the replicating BKV? * Differences in immune response according to viral replication profile: Does the immune response differ between patients presenting isolated BKV viruria and those with both viruria and viremia? * Can new biomarkers help predict BKV replication and viremia? Patients will be grouped according to their BKV replication profile: Group 1: patients with BKV viruria without viremia Group 2: patients with both BKV viruria and viremia Comparisons between these two groups will help identify whether different viral genotypes or immune responses are associated with systemic dissemination (viremia). Kidney transplant recipients will be included if they present BKV viruria during their post-transplant follow-up. Additional blood samples will be collected during scheduled follow-up visits at the university hospital. These visits are part of routine clinical care, and no extra visits will be required specifically for the study.

L'objectif de cette étude observationnelle est de caractériser la réplication urinaire du polyomavirus BK (BKV) chez les receveurs de transplantation rénale. Bien que la réactivation du BKV après transplantation soit bien établie, l'origine du virus en réplication demeure incertaine. Les données actuelles suggèrent que le BKV détecté chez les receveurs pourrait provenir soit du rein transplanté (d'origine donneur), soit d'une réactivation virale chez le receveur. L'évaluation de nouveaux biomarqueurs permettant de prédire la réplication du BKV est nécessaire. Cette étude vise à répondre aux questions clés suivantes : * Origine du virus en réplication : le BKV détecté chez le receveur est-il identique au virus provenant du rein du donneur ? * Réponse immunitaire de l'hôte et génotype viral : existe-t-il une association entre la réponse immunitaire du receveur et le génotype du BKV en réplication ? * Différences de réponse immunitaire selon le profil de réplication virale : la réponse immunitaire diffère-t-elle entre les patients présentant une virurie à BKV isolée et ceux présentant à la fois une virurie et une virémie ? * De nouveaux biomarqueurs peuvent-ils aider à prédire la réplication du BKV et la virémie ? Les patients seront répartis selon leur profil de réplication du BKV : Groupe 1 : patients présentant une virurie à BKV sans virémie ; Groupe 2 : patients présentant à la fois une virurie et une virémie à BKV. Les comparaisons entre ces deux groupes permettront de déterminer si différents génotypes viraux ou réponses immunitaires sont associés à une dissémination systémique (virémie). Les receveurs de transplantation rénale seront inclus s'ils présentent une virurie à BKV au cours de leur suivi post-transplantation. Des prélèvements sanguins supplémentaires seront effectués lors des visites de suivi programmées au centre hospitalo-universitaire. Ces visites font partie du suivi clinique habituel, et aucune visite supplémentaire ne sera nécessaire spécifiquement pour l'étude.
