Characterization of the Intrahepatic Inflammatory Microenvironment in Patients With Non-alcoholic Steatohepatitis by Transcriptomics, Immunophenotyping and Functional Proteomics: Profil-NASH

Non-alcoholic fatty liver disease (NAFLD) is a nosological entity that groups together non-alcoholic fatty liver (NAFL) and non-alcoholic steatohepatitis (NASH). Unlike NAFL, NASH is characterized by intrahepatic inflammation, and is solely at risk of progression to cirrhosis and hepatocellular carcinoma (HCC). It is currently estimated that NAFLD affects approximately 25% of the world's adult population, and its incidence is rising in all regions of the world. Nevertheless, of all patients with NAFLD, only \~25% have NASH. Identifying patients with NASH is therefore crucial, determining the need for follow-up to detect the onset of fibrosis and/or HCC, and eventual access to therapeutic trials. Furthermore, intrahepatic inflammation, the initial driver of NASH, appears to play an important role in the development of fibrosis and HCC, which can occur in the absence of cirrhosis in these patients. However, few studies have been carried out in humans to date, with data mainly coming from mouse models. An innovative technique, Fine-Needle Aspiration (FNA), enables to obtain cells from the liver compartment, including large numbers of immune cells. In participants with NAFLD and indication of liver biopsy, a FNA will also be performed. Forty patients will be included, with \~75% of NASH and \~25% of NAFL expected. The investigators will study the phenotypic and functional characteristics of human intrahepatic inflammatory cells obtained by the FNA with different innovative techniques (RNAseq, multiparameter immunophenotyping, single-cell secretome and phosphoproteome). Peripheral Blood Mononuclear Cells and circulating microRNAs, known to regulate immune responses, will also be analysed. The hypothesis of Profile-NASH is that intrahepatic inflammatory profiles differ between NASH and NAFL, and is associated with fibrosis progression and carcinogenesis. This pilot study, based on high-definition technologies, will provide precise new insights into the quality of intrahepatic inflammation and the mechanisms favoring the transition from NAFL to NASH and its progression. Precise analysis of the intrahepatic inflammatory microenvironment will enable the investigators to identify new players in the pathogenesis of NASH, and potential future therapeutic targets.

La stéatose hépatique non alcoolique (NAFLD) est une entité nosologique regroupant la stéatose hépatique non alcoolique simple (NAFL) et la stéatohépatite non alcoolique (NASH). Contrairement à la NAFL, la NASH se caractérise par une inflammation intrahépatique et est seule à risque d'évoluer vers la cirrhose et le carcinome hépatocellulaire (CHC). On estime actuellement que la NAFLD touche environ 25 % de la population adulte mondiale, et son incidence est en augmentation dans toutes les régions du monde. Néanmoins, parmi l'ensemble des patients atteints de NAFLD, seuls ~25 % présentent une NASH. Identifier les patients atteints de NASH est donc essentiel, afin de déterminer le besoin de suivi pour détecter l'apparition d'une fibrose et/ou d'un CHC, ainsi que l'éventuel accès à des essais thérapeutiques. Par ailleurs, l'inflammation intrahépatique, moteur initial de la NASH, semble jouer un rôle important dans le développement de la fibrose et du CHC, qui peut survenir en l'absence de cirrhose chez ces patients. Cependant, peu d'études ont été réalisées chez l'humain à ce jour, les données provenant principalement de modèles murins. Une technique innovante, la cytoponction à l'aiguille fine (FNA), permet d'obtenir des cellules du compartiment hépatique, y compris un grand nombre de cellules immunitaires. Chez les participants atteints de NAFLD et présentant une indication de biopsie hépatique, une FNA sera également réalisée. Quarante patients seront inclus, avec environ 75 % de NASH et environ 25 % de NAFL attendus. Les investigateurs étudieront les caractéristiques phénotypiques et fonctionnelles des cellules inflammatoires intrahépatiques humaines obtenues par FNA, à l'aide de différentes techniques innovantes (RNAseq, immunophénotypage multiparamétrique, sécrétome et phosphoprotéome en cellule unique). Les cellules mononucléées du sang périphérique et les microARN circulants, connus pour réguler les réponses immunitaires, seront également analysés. L'hypothèse de Profil-NASH est que les profils inflammatoires intrahépatiques diffèrent entre la NASH et la NAFL, et sont associés à la progression de la fibrose et à la carcinogenèse. Cette étude pilote, fondée sur des technologies de haute définition, apportera de nouvelles données précises sur la qualité de l'inflammation intrahépatique et sur les mécanismes favorisant la transition de la NAFL vers la NASH et sa progression. Une analyse précise du microenvironnement inflammatoire intrahépatique permettra aux investigateurs d'identifier de nouveaux acteurs de la physiopathologie de la NASH, ainsi que de potentielles futures cibles thérapeutiques.
