Thrombin Generation and Prediction of Thromboembolic Events in Oncology Patients at Risk
Coagulation is a complex system which, through the action of thrombin, leads to the formation of fibrin, which stabilises the platelet clot. Any disturbance in the balance between procoagulant and anticoagulant factors can tip the physiological process either towards a state of hypercoagulability leading to thrombosis or hypocoagulability responsible for bleeding. Due to a number of factors, cancer is associated with a state of hypercoagulability, leading to thrombosis. The incidence of venous thromboembolism (VTE) in cancer patients varies from 15 to 20% depending on the type of cancer, the stage of the disease and the associated treatments (ONCORIF data, November 2021). The risk of venous thromboembolism (VTE) is greatly increased in cancer patients (RR x 3 to 6) and doubled in the case of associated chemotherapy (1). VTE is a poor prognostic factor, occurs mainly in the first 6 months after diagnosis and is the second leading cause of death in cancer patients. At present, haemostasis tests performed in medical laboratories independently explore the different coagulation pathways but do not allow the overall haemostatic profile of a hyper- or hypocoagulable patient to be assess. Based on this knowledge base, the aim of our study will be to monitor thrombogram profiles during the management of patients with tumours at high risk of thromboembolism (lung, pancreas, stomach, glioblastoma) and to correlate these profiles with the risk of a thromboembolic event occurring in these patients. The aim of the project is to validate a simple predictive test (suitable for clinical use) for the risk of thromboembolism in these patients. These analyses will also make it possible to monitor the impact of chemotherapy on changes in the thrombin generation test in patients.
La coagulation est un système complexe qui, par l'action de la thrombine, conduit à la formation de fibrine, laquelle stabilise le clou plaquettaire. Toute perturbation de l'équilibre entre facteurs procoagulants et anticoagulants peut faire basculer le processus physiologique soit vers un état d'hypercoagulabilité conduisant à une thrombose, soit vers une hypocoagulabilité responsable d'hémorragies. En raison de nombreux facteurs, le cancer est associé à un état d'hypercoagulabilité, conduisant à une thrombose. L'incidence de la maladie thromboembolique veineuse (VTE) chez les patients atteints de cancer varie de 15 à 20% selon le type de cancer, le stade de la maladie et les traitements associés (données ONCORIF, novembre 2021). Le risque de maladie thromboembolique veineuse (VTE) est très augmenté chez les patients atteints de cancer (RR x 3 à 6) et doublé en cas de chimiothérapie associée (1). La VTE est un facteur de mauvais pronostic, survient principalement dans les 6 premiers mois après le diagnostic et constitue la deuxième cause de décès chez les patients atteints de cancer. Actuellement, les tests d'hémostase réalisés dans les laboratoires médicaux explorent de façon indépendante les différentes voies de la coagulation, mais ne permettent pas d'évaluer le profil hémostatique global d'un patient hyper- ou hypocoagulable. Sur la base de ces connaissances, l'objectif de notre étude sera de suivre les profils de thrombinogramme au cours de la prise en charge de patients porteurs de tumeurs à haut risque thromboembolique (poumon, pancréas, estomac, glioblastome) et de corréler ces profils avec le risque de survenue d'un événement thromboembolique chez ces patients. L'objectif du projet est de valider un test prédictif simple (adapté à un usage clinique) du risque thromboembolique chez ces patients. Ces analyses permettront également de suivre l'impact de la chimiothérapie sur l'évolution du test de génération de thrombine chez les patients.
