Quantiferon CMV to Identify Treatment Need for Asymptomatic CMV Infection After Solid Organ Transplant (QUANTIFOT)
Use of QuantiFERON® CMV in the Therapeutic Decision in Asymptomatic CMV Infection in Solid Organ Transplant Recipients
Context Cytomegalovirus (CMV) infection is a frequent and potentially severe event in solid organ transplant (SOT) recipients. Most of available treatment display adverse effects that limit their use. Therefore, in case of an infection, it is of primary importance to identify the patients at high risk of severe infection and/or disease, and who ill benefit the most from antiviral therapy. As CMV infection is mainly controlled by cellular immunity, measuring specific anti-CMV T lymphocyte immunity could be an interesting tool for identifying these at-risk individuals. One of these tests is the QuantiFERON-CMV (QF-CMV) assay (QuiagenTM, Courtabœuf, France). Aim of the study The aim of the study is to determine the extent to which the QF-CMV can be use to identify, among SOT recipients with a CMV viremia, those that may not need antiviral therapy. Methods Participation to the study will be proposed to SOT recipients with an asymptomatic CMV infection with a blood viral load between 1,000 and 15,000 IU/mL. The QF-CMV will be performed in included participants, and the result will be given or not to the clinician in charge (according to the attributed group through randomisation). * In the group without result communication, the clinician in charge will determine whether a treatment is needed according to the guidelines and the local practices. * in the group with result communication, the clinician in charge will be advised not to introduce antiviral therapy if the result is positive, and to determine whether a treatment is needed according to the guidelines and the local practices if the result is positive. In the following weeks, the viral load will be monitored, along with creatininemia, cell blood count, and kalemia (to detect antiviral adverse effect). The participants will be sampled: * 5 to 12 days after QF-CMV sampling (V2) ; * 7 to 14 days days after V2 (V3 - between D12 and D26) ; * 7 to 14 days days after V3 (V4 - between D19 and D40) . Endpoints The primary endpoint is the rate of uncontrolled infection 5 to 12 days after QF-CMV sampling, defined as follows: * Blood CMV viral load \>10,000 IU/mL \[4 log\]; * And/or increase in blood viral load ≥0.5 log IU/mL with CV otherwise \>5000 IU/mL; * And/or the onset of CMV disease. The secondary endpoint is the is the occurrence antiviral adverse effects (hematoxicity or nephrotoxicity).
Contexte L'infection à cytomégalovirus (CMV) est un événement fréquent et potentiellement sévère chez les receveurs de transplantation d'organe solide (TOS). La plupart des traitements disponibles présentent des effets indésirables qui limitent leur utilisation. Il est donc primordial, en cas d'infection, d'identifier les patients à haut risque d'infection et/ou de maladie sévère, et qui bénéficieront le plus d'un traitement antiviral. Comme l'infection à CMV est principalement contrôlée par l'immunité cellulaire, la mesure de l'immunité lymphocytaire T anti-CMV spécifique pourrait constituer un outil intéressant pour identifier ces individus à risque. L'un de ces tests est le dosage QuantiFERON-CMV (QF-CMV) (QuiagenTM, Courtabœuf, France). Objectif de l'étude L'objectif de l'étude est de déterminer dans quelle mesure le QF-CMV peut être utilisé pour identifier, parmi les receveurs de TOS présentant une virémie à CMV, ceux qui pourraient ne pas nécessiter de traitement antiviral. Méthodes La participation à l'étude sera proposée aux receveurs de TOS présentant une infection à CMV asymptomatique avec une charge virale sanguine comprise entre 1 000 et 15 000 UI/mL. Le QF-CMV sera réalisé chez les participants inclus, et le résultat sera communiqué ou non au clinicien en charge (selon le groupe attribué par randomisation). * Dans le groupe sans communication du résultat, le clinicien en charge déterminera si un traitement est nécessaire selon les recommandations et les pratiques locales. * Dans le groupe avec communication du résultat, le clinicien en charge sera invité à ne pas instaurer de traitement antiviral si le résultat est positif, et à déterminer si un traitement est nécessaire selon les recommandations et les pratiques locales si le résultat est positif. Au cours des semaines suivantes, la charge virale sera surveillée, ainsi que la créatininémie, la numération de la formule sanguine et la kaliémie (pour détecter un effet indésirable antiviral). Les participants seront prélevés : * 5 à 12 jours après le prélèvement du QF-CMV (V2) ; * 7 à 14 jours après V2 (V3 - entre J12 et J26) ; * 7 à 14 jours après V3 (V4 - entre J19 et J40). Critères de jugement Le critère de jugement principal est le taux d'infection non contrôlée 5 à 12 jours après le prélèvement du QF-CMV, défini comme suit : * Charge virale sanguine du CMV > 10 000 UI/mL [4 log] ; * Et/ou augmentation de la charge virale sanguine ≥ 0,5 log UI/mL avec une CV par ailleurs > 5 000 UI/mL ; * Et/ou la survenue d'une maladie à CMV. Le critère de jugement secondaire est la survenue d'effets indésirables antiviraux (hématotoxicité ou néphrotoxicité).
