The combination of amoxicillin-clavulanate (AC) and a fluoroquinolone (FQ) is currently recommended for the treatment for outpatients with hematologic malignancies presenting with chemotherapy-induced fever (CIF), if the expected duration of neutropenia is \< 7 days. However, infections due to Pseudomonas aeruginosa (naturally resistant to AC) are rare in this population. Furthermore, FQ might result in severe adverse events, and to the selection of bacterial resistance. The investigators hypothesize that monotherapy with AC is non-inferior to the reference treatment AC + FQ in the outpatient treatment of CIF in adult hematology patients. Method: Pragmatic, multicentre, randomized clinical trial, controlled in two parallel groups, with stratified randomization according to the type of haematological disorder. The study will include 1,526 adult patients with one of the following haematological disorders: (1) lymphoma of all histology types treated with the goal of remission; (2) myelodysplasia treated with azacytidine; (3) acute myeloblastic leukemia receiving non-intensive care, with a basal neutrophils count \> 1000/mm3. The participants will be randomized prior to chemotherapy to receive either Amoxicillin-clavulanate 1g/152 mg tid. (AC) or Amoxicillin-clavulanate 1g/152 mg tid. and Ciprofloxacin 500 mg bid. (AC+C) orally for 7 days, to be taken in case of CIF. Only the first episode of CIF of each patient will be included in the analysis. The main evaluation criterion will be clinical success, defined as apyrexia 4 days after the first antibiotic dose, without modification of antibiotic treatment. The main secondary evaluation criteria (recorded at Day 14) will be fever recurrence, hospital admission, modification of antibiotic treatment, treatment with a beta-lactamine antibiotic efficient against P. aeruginosa, duration of antibiotic treatment, bacteraemia, inefficiency of the study treatment against the identified bacteria, and adverse events of FQ.
L'association amoxicilline-acide clavulanique (AC) et d'une fluoroquinolone (FQ) est actuellement recommandée pour le traitement ambulatoire des patients atteints d'hémopathies malignes présentant une fièvre induite par la chimiothérapie (FIC), lorsque la durée attendue de la neutropénie est < 7 jours. Cependant, les infections dues à Pseudomonas aeruginosa (naturellement résistant à l'AC) sont rares dans cette population. Par ailleurs, les FQ peuvent entraîner des événements indésirables graves et favoriser la sélection de résistances bactériennes. Les investigateurs font l'hypothèse que la monothérapie par AC est non-inférieure au traitement de référence AC + FQ dans le traitement ambulatoire de la FIC chez des patients adultes en hématologie. Méthode : essai clinique pragmatique, multicentrique, randomisé, contrôlé, à deux groupes parallèles, avec randomisation stratifiée selon le type d'hémopathie. L'étude inclura 1 526 patients adultes présentant l'une des hémopathies suivantes : (1) lymphome, tous types histologiques confondus, traité dans un objectif de rémission ; (2) myélodysplasie traitée par azacitidine ; (3) leucémie aiguë myéloblastique traitée par soins non intensifs, avec un taux basal de polynucléaires neutrophiles > 1 000/mm3. Les participants seront randomisés avant la chimiothérapie pour recevoir soit de l'amoxicilline-acide clavulanique 1 g/152 mg 3 fois par jour (AC), soit de l'amoxicilline-acide clavulanique 1 g/152 mg 3 fois par jour associée à la ciprofloxacine 500 mg 2 fois par jour (AC+C), par voie orale pendant 7 jours, à prendre en cas de FIC. Seul le premier épisode de FIC de chaque patient sera inclus dans l'analyse. Le critère d'évaluation principal sera le succès clinique, défini comme l'apyrexie à 4 jours après la première dose d'antibiotique, sans modification du traitement antibiotique. Les principaux critères d'évaluation secondaires (recueillis au jour 14) seront la récidive de la fièvre, l'hospitalisation, la modification du traitement antibiotique, le recours à un antibiotique bêta-lactamine efficace contre P. aeruginosa, la durée du traitement antibiotique, la bactériémie, l'inefficacité du traitement de l'étude contre la bactérie identifiée, et les événements indésirables liés aux FQ.
