Evaluation of the Impact of Self-rehabilitation After Foot Surgery Via a Connected Application

Hallux valgus is the most common forefoot deformity and primarily affects women. Surgical treatment involves an osteotomy of the first metatarsal to reduce the M1-M2 angle. Often, an osteotomy of the first phalanx of the big toe is performed in conjunction to reduce the M1-P1 angle. Once the deformity has been corrected, the second objective is to restore the foot's function, particularly mobility and plantar flexion strength. To achieve this, rehabilitation is prescribed, but inconsistently due to the lack of clear guidelines and limited patient participation, especially outside of the physiotherapist's scheduled appointments. Continuous passive range of motion exercises can accelerate the return to standard footwear and normal joint mobility. This study is based on the hypothesis that an application connected with rehabilitation exercises to be done outside of sessions with a physiotherapist reproduces this mechanism of continuous passive mobility. In this context, it is hoped that patients benefiting from self-rehabilitation of the foot via the connected application, in addition to traditional care with a physiotherapist, will achieve better postoperative mobility of the metatarsophalangeal joint of the hallux compared to traditional care with a physiotherapist in patients undergoing hallux valgus surgery, with an isolated procedure on the first ray. The main objective is to compare the mobility of the metatarsophalangeal joint of the hallux 6 months after an operation for isolated hallux valgus by osteotomy in patients benefiting from optimized rehabilitation with a physiotherapist using a connected application compared to patients receiving standard care with a physiotherapist.

L'hallux valgus est la déformation de l'avant-pied la plus fréquente et touche principalement les femmes. Le traitement chirurgical consiste en une ostéotomie du premier métatarsien afin de réduire l'angle M1-M2. Une ostéotomie de la première phalange du gros orteil est souvent réalisée en complément afin de réduire l'angle M1-P1. Une fois la déformation corrigée, le second objectif est de restaurer la fonction du pied, en particulier la mobilité et la force de flexion plantaire. Pour ce faire, une rééducation est prescrite, mais de façon inconstante, faute de recommandations claires et en raison d'une participation limitée des patients, notamment en dehors des rendez-vous programmés avec le kinésithérapeute. Des exercices continus de mobilisation passive peuvent accélérer le retour au chaussage standard et à une mobilité articulaire normale. Cette étude repose sur l'hypothèse qu'une application connectée proposant des exercices de rééducation à réaliser en dehors des séances avec un kinésithérapeute reproduit ce mécanisme de mobilité passive continue. Dans ce contexte, il est attendu que les patients bénéficiant d'une autorééducation du pied via l'application connectée, en complément de la prise en charge habituelle par un kinésithérapeute, obtiennent une meilleure mobilité postopératoire de l'articulation métatarsophalangienne de l'hallux, par comparaison avec la prise en charge habituelle par un kinésithérapeute, chez des patients opérés d'un hallux valgus par un geste isolé sur le premier rayon. L'objectif principal est de comparer la mobilité de l'articulation métatarsophalangienne de l'hallux 6 mois après une intervention pour hallux valgus isolé par ostéotomie chez des patients bénéficiant d'une rééducation optimisée avec un kinésithérapeute à l'aide d'une application connectée, par rapport à des patients recevant une prise en charge standard avec un kinésithérapeute.
