A Randomized Phase III Trial With a Factorial Design Evaluating the Efficacy and Safety of Darolutamide and Stereotactic Dose Escalated Radiotherapy in Patients With Localized Prostate Cancer and High-risk Features of Relapse, From the Prostate Cancer Consortium in Europe (PEACE)

PEACE 7 is an international, multicenter, randomized, open-label phase III study that aims at evaluating the efficacy and safety of darolutamide and of stereotactic dose escalated prostate radiotherapy in patients with localised prostate cancer and high-risk features of relapse (defined as patients with at least 2 high-risk criteria from National Comprehensive Cancer Network (NCCN) classification) using a factorial (2x2) design. The primary objective of this study is to assess the efficacy of darolutamide and of a stereotactic dose escalated radiotherapy targeting prostate in combination with ADT and pelvic nodal radiotherapy in terms of metastasis-free survival (MSF). Patients will be randomized (1:1:1:1) to receive either: * Arm A (Standard arm): ADT + conventional fractionated or moderately hypo-fractionated prostate radiotherapy including pelvic nodal radiotherapy * Arm B (Experimental arm): ADT + conventional fractionated or moderately hypo-fractionated prostate radiotherapy including pelvic nodal radiotherapy + darolutamide * Arm C (Experimental arm): ADT + conventional fractionated or moderately hypo-fractionated pelvic nodal radiotherapy + Prostate SBRT * Arm D (Experimental arm): ADT + conventional fractionated or moderately hypo-fractionated pelvic nodal radiotherapy + Prostate SBRT + darolutamide Patient will receive systemic treatments (ADT and/or darolutamide) during 2 years where visits on site are planned at D45, D90, D180 and then every 3 months for checkups and follow prostate specific antigen (PSA) level. Metastasis-free survival (MFS) is defined as the time interval from randomization to the date of the appearance of metastasis (on next generation imaging) or death (from any cause), whichever occurs first. Radiographic evaluation will be carried out at the time of biochemical failure (Phoenix criteria) or in case of clinical suspicion. After biochemical failure (Phoenix criteria) radiographic evaluation on next generation imaging (prostate-specific membrane antigen (PSMA) positron emission tomography (PET) scan (any European Medicines Agency (EMA) approved PSMA tracer)) will be performed every 6 months until a metastatic site of relapse is identified and will be repeated at each subsequent PSA progression.

PEACE 7 est une étude internationale, multicentrique, randomisée, ouverte, de phase III, qui vise à évaluer l'efficacité et la tolérance du darolutamide et d'une radiothérapie prostatique stéréotaxique à dose escaladée chez des patients atteints d'un cancer de la prostate localisé présentant des caractéristiques de haut risque de rechute (définis comme des patients présentant au moins 2 critères de haut risque selon la classification du National Comprehensive Cancer Network (NCCN)), selon un plan factoriel (2x2). L'objectif principal de cette étude est d'évaluer l'efficacité du darolutamide et d'une radiothérapie stéréotaxique à dose escaladée ciblant la prostate, en association avec un traitement de privation androgénique (ADT) et une radiothérapie ganglionnaire pelvienne, en termes de survie sans métastase (SSM). Les patients seront randomisés (1:1:1:1) pour recevoir : * Bras A (bras standard) : ADT + radiothérapie prostatique par fractionnement conventionnel ou modérément hypofractionné, incluant une radiothérapie ganglionnaire pelvienne * Bras B (bras expérimental) : ADT + radiothérapie prostatique par fractionnement conventionnel ou modérément hypofractionné, incluant une radiothérapie ganglionnaire pelvienne + darolutamide * Bras C (bras expérimental) : ADT + radiothérapie ganglionnaire pelvienne par fractionnement conventionnel ou modérément hypofractionné + radiothérapie stéréotaxique prostatique (SBRT) * Bras D (bras expérimental) : ADT + radiothérapie ganglionnaire pelvienne par fractionnement conventionnel ou modérément hypofractionné + SBRT prostatique + darolutamide Les patients recevront des traitements systémiques (ADT et/ou darolutamide) pendant 2 ans, avec des visites sur site prévues à J45, J90, J180, puis tous les 3 mois pour des bilans et un suivi du taux d'antigène prostatique spécifique (PSA). La survie sans métastase (SSM) est définie comme l'intervalle de temps entre la randomisation et la date d'apparition d'une métastase (à l'imagerie de nouvelle génération) ou du décès (toutes causes confondues), selon l'événement survenant en premier. Une évaluation radiographique sera réalisée au moment de l'échec biochimique (critères de Phoenix) ou en cas de suspicion clinique. Après un échec biochimique (critères de Phoenix), une évaluation radiographique par imagerie de nouvelle génération (tomographie par émission de positons (TEP) à l'antigène membranaire spécifique de la prostate (PSMA), avec tout traceur PSMA approuvé par l'Agence européenne des médicaments (EMA)) sera réalisée tous les 6 mois jusqu'à l'identification d'un site métastatique de rechute, et sera répétée à chaque progression ultérieure du PSA.
