Trial of FOLFIRI + Zimberelimab + Domvanalimab vs FOLFIRI With a Hybrid Synthetic Control Arm in Second Line Treatment of Neuroendocrine Carcinoma of Gastro-enteropancreatic or Unknown Origin (REWENEC 01)

PRODIGE 113 (FFCD 2314) - REWENEC 01 STUDY Randomized Trial of FOLFIRI + Zimberelimab + Domvanalimab vs FOLFIRI With a Hybrid Synthetic Control Arm in Second Line Treatment of Neuroendocrine Carcinoma of Gastro-enteropancreatic or Unknown Origin. Phase II Comparative Randomized Study - Multicentric

Background Neuroendocrine carcinomas (NECs) of gastro-entero-pancreatic (GEP) or unknown (UK) origin are rare and highly aggressive diseases. The recommended first-line (L1) treatment is platinum-etoposide combination therapy, which has a progression-free survival (PFS) of only 4-9 months and a median overall survival (OS) of approximately 12 months. All patients experience relapse, often rapidly after this first line of chemotherapy. The standard second-line (L2) chemotherapies recommended by ESMO, ENETS, and NCCN, FOLFIRI and FOLFOX, have modest efficacy with a PFS of 3 months and a median OS of 6 months. The BEVANEC study (PHRCK 2014, NCT02820857) reported no benefit of FOLFIRI + bevacizumab compared to FOLFIRI in a randomized phase II study that enrolled 150 patients in 26 centers over a period of 5 years in France. To date, the most promising efficacy data for this highly aggressive cancer come from clinical trials of immune checkpoint inhibitors (ICIs) targeting the PD-1/PD-L1 checkpoint. For example, in France, the non-comparative phase II NIPINEC trial (NCT03591731) randomized patients to receive nivolumab +/- ipilimumab in L2/3 and achieved its primary evaluation criterion (ORR-8 weeks\>10%). Other trials in Europe and worldwide have also reported efficacy data in the context of single-arm studies. Scientific Questions and Unmet Needs: 1. New therapeutic options/perspectives are necessary for patients with GEP/UK NECs given the limited overall survival. 2. Approximately 50% of patients experienced early progression under immunotherapy in the NIPINEC trial and other trials, which may be explained by the absence of chemotherapy combined with immunotherapy and/or the existence of resistance mechanisms. 3. In the GEP/UK NEC indication, the design of these immunotherapy trials has been non-comparative single-arm studies because the realization of randomized comparative trials is considered very difficult for these very rare cancers (incidence \<5/million). Rationale for the REWENEC-01 Trial The DURIGAST PRODIGE 59 study, conducted by the FFCD, demonstrated the feasibility and safety of the FOLFIRI + double immune checkpoint inhibitor (anti-PD-1 and anti-CTLA4) combination, as well as for the combination Folfox-Domvanalimab-Zimberelimab (anti-PD-1 and anti-TIGIT). In a translational study of the immune phenotype in patients with NECs treated with the anti-PD1 pembrolizumab, an increase in TIGIT expression was observed after pembrolizumab treatment and higher TIGIT expression on T cells in the blood of patients with high Ki67 expression in their tumors. These data suggest that TIGIT is a potential complementary therapeutic target to PD-1/PD-L1 checkpoint inhibition in GEP/UK NECs. Domvanalimab has been developed as an anti-TIGIT monoclonal antibody and zimberelimab as an anti-PD-1. Design and primary objective of the REWNEC-01 Trial The REWENEC-01 trial is a comparative phase II trial that will randomize GEP/UK NEC patients between an experimental arm FOLFIRI+Zimberelimab + Domvanalimab and a control arm FOLFIRI in L2. The FOLFIRI arm will be a "hybrid" synthetic control arm composed of patients from historical/external data from the FOLFIRI arm of BEVANEC and French retrospective studies RBNEC and CEPD, mixed with patients recruited prospectively during the trial and randomized to the control arm. The randomization ratio for patients included prospectively during the trial will be 4:1 (4 patients assigned to FOLFIRI+Zimberelimab + Domvanalimab for 1 patient assigned to FOLFIRI). The randomization algorithm will take into account "external" patients assigned progressively to the control arm to obtain a 1:1 ratio between the trial arms, with balanced distributions of stratification factors between the two arms. With 77 patients to be included, this strategy will provide statistical power equivalent to that of a trial including 122 patients, sufficient to demonstrate an advantage in overall survival rate at 12 months from 32%

Contexte Les carcinomes neuroendocrines (CNE) d'origine gastro-entéro-pancréatique (GEP) ou de primitif inconnu (PI) sont des maladies rares et hautement agressives. Le traitement de première ligne (L1) recommandé est une association platine-étoposide, qui n'offre qu'une survie sans progression (SSP) de 4 à 9 mois et une survie globale (SG) médiane d'environ 12 mois. Tous les patients rechutent, souvent rapidement après cette première ligne de chimiothérapie. Les chimiothérapies standard de deuxième ligne (L2) recommandées par l'ESMO, l'ENETS et le NCCN, le FOLFIRI et le FOLFOX, présentent une efficacité modeste avec une SSP de 3 mois et une SG médiane de 6 mois. L'étude BEVANEC (PHRCK 2014, NCT02820857) n'a rapporté aucun bénéfice du FOLFIRI + bévacizumab par rapport au FOLFIRI dans une étude randomisée de phase II ayant inclus 150 patients dans 26 centres sur une période de 5 ans en France. À ce jour, les données d'efficacité les plus prometteuses pour ce cancer hautement agressif proviennent des essais cliniques d'inhibiteurs des points de contrôle immunitaires (ICI) ciblant le point de contrôle PD-1/PD-L1. Par exemple, en France, l'essai de phase II non comparatif NIPINEC (NCT03591731) a randomisé des patients pour recevoir du nivolumab +/- ipilimumab en L2/3 et a atteint son critère d'évaluation principal (taux de réponse objective à 8 semaines > 10 %). D'autres essais menés en Europe et dans le monde ont également rapporté des données d'efficacité dans le cadre d'études à bras unique. Questions scientifiques et besoins non couverts : 1. De nouvelles options/perspectives thérapeutiques sont nécessaires pour les patients atteints de CNE GEP/PI compte tenu de la survie globale limitée. 2. Environ 50 % des patients ont présenté une progression précoce sous immunothérapie dans l'essai NIPINEC et d'autres essais, ce qui peut s'expliquer par l'absence de chimiothérapie associée à l'immunothérapie et/ou par l'existence de mécanismes de résistance. 3. Dans l'indication des CNE GEP/PI, la conception de ces essais d'immunothérapie a reposé sur des études à bras unique non comparatives, car la réalisation d'essais randomisés comparatifs est considérée comme très difficile pour ces cancers très rares (incidence < 5/million). Justification de l'essai REWENEC-01 L'étude DURIGAST PRODIGE 59, menée par la FFCD, a démontré la faisabilité et la sécurité de l'association FOLFIRI + double inhibiteur de point de contrôle immunitaire (anti-PD-1 et anti-CTLA4), ainsi que de l'association Folfox-domvanalimab-zimberelimab (anti-PD-1 et anti-TIGIT). Dans une étude translationnelle du phénotype immunitaire chez des patients atteints de CNE traités par le pembrolizumab (anti-PD1), une augmentation de l'expression de TIGIT a été observée après traitement par pembrolizumab, ainsi qu'une expression plus élevée de TIGIT sur les lymphocytes T dans le sang des patients présentant une forte expression de Ki67 dans leurs tumeurs. Ces données suggèrent que TIGIT constitue une cible thérapeutique complémentaire potentielle à l'inhibition du point de contrôle PD-1/PD-L1 dans les CNE GEP/PI. Le domvanalimab a été développé comme anticorps monoclonal anti-TIGIT et le zimberelimab comme anti-PD-1. Schéma et objectif principal de l'essai REWENEC-01 L'essai REWENEC-01 est un essai comparatif de phase II qui randomisera les patients atteints de CNE GEP/PI entre un bras expérimental FOLFIRI + zimberelimab + domvanalimab et un bras contrôle FOLFIRI en L2. Le bras FOLFIRI sera un bras contrôle synthétique « hybride », composé de patients issus de données historiques/externes du bras FOLFIRI de BEVANEC et des études rétrospectives françaises RBNEC et CEPD, associés à des patients recrutés prospectivement au cours de l'essai et randomisés dans le bras contrôle. Le ratio de randomisation pour les patients inclus prospectivement au cours de l'essai sera de 4:1 (4 patients affectés au bras FOLFIRI + zimberelimab + domvanalimab pour 1 patient affecté au bras FOLFIRI).
